MediShield Life and MediSave: how Singapore pays for healthcare

Singapore pays for healthcare through insurance, savings and subsidies working together. How MediShield Life, MediSave and Integrated Shield Plans fit.

Singapore's healthcare financing combines government subsidies, compulsory savings and insurance. For most residents, two names matter most: MediSave, your medical savings account in CPF, and MediShield Life, the national health insurance scheme. Understanding how they fit together helps you avoid surprises when a large bill arrives. This guide explains how MediShield Life and MediSave work, what Integrated Shield Plans add, and how a hospital bill is paid.

Singapore's layers of healthcare financing

Singapore's approach is often described as layered. Government subsidies reduce bills at public hospitals and polyclinics, especially in subsidised wards. MediShield Life covers large bills. MediSave lets you pay your share from savings rather than cash. MediFund is a safety net for those who still cannot afford their bills. Each layer catches costs the previous one leaves behind.

MediShield Life

MediShield Life is basic health insurance that covers all Singapore citizens and permanent residents for life, including those with pre-existing conditions. It is designed for large hospital bills and certain costly outpatient treatments, such as dialysis and chemotherapy, at the level of subsidised wards in public hospitals.

Like most insurance, it does not pay the whole bill. You first pay a deductible, a fixed amount per year, and then a co-insurance share of the remaining bill, while MediShield Life pays the rest up to claim limits. Premiums depend on age and are fully payable from MediSave, so most people do not need to pay them in cash. Government subsidies help lower-income households and seniors with premiums.

MediSave

MediSave is the part of your CPF contributions set aside for healthcare. It can be used to pay MediShield Life and Integrated Shield Plan premiums, deductibles and co-insurance for hospital stays, and approved outpatient treatments, within limits set for each type of treatment. You can also use your MediSave to help pay for your immediate family members' healthcare. MediSave earns at least 4% interest a year, the same as the Special and Retirement Accounts; see our guide to CPF interest rates.

There is a cap on how much MediSave you can hold, known as the Basic Healthcare Sum. Contributions above it flow into your other CPF accounts, so building MediSave also supports your retirement savings.

Integrated Shield Plans

If you want cover for higher-class wards or private hospitals, you can buy an Integrated Shield Plan from a private insurer. It combines MediShield Life with additional private cover. Part of the premium can be paid from MediSave, up to a yearly withdrawal limit, with the rest in cash. Optional riders can cover the deductible and co-insurance, but they must be paid in cash and generally come with their own co-payment rules. Premiums rise with age, so check that you can afford the plan in later life as well as now.

Putting it together: paying a hospital bill

When you are hospitalised, government subsidies reduce the bill first, then MediShield Life or your Integrated Shield Plan pays its share, and the remainder can largely come from MediSave. MediFund provides further help for those who still cannot afford their bills. Checking your MediSave balance and the terms of any private plan before you need them is the best way to avoid stress later. See how much goes into MediSave from your pay in our CPF accounts guide.

Planning for healthcare costs

Even with insurance, outpatient bills, medicines and some treatments may need cash. Keeping an emergency fund in a safe, accessible place such as Singapore Savings Bonds or a savings account helps. Our Singapore income tax and CPF calculator shows your CPF contributions, including the share that goes into MediSave.
